REGISTER WITH THE PRACTICE?
If you live in the practice area and wish to register, please bring your medical card, or fill in a registration form, which requires details of your previous doctor. You will be given an appointment with the practice nurse for a registration medical check. Providing you live within the practice area, you will be registered without discrimination.
